FIELD CASE MANAGER RN

NCP is seeking an RN interested in Field Case Management along with clinical RN care.  Candidates must be detail and deadline oriented, possess the ability to work closely with other non-clinical and clinical team members, have a positive attitude and be very reliable. The perfect candidate will be self motivated and have excellent ability to provide leadership for the company, maintain a qualified staff that will meet patient needs and successfully execute company initiatives. NCP strives to find someone who will be a resource to the leadership team regarding the needs of the community, personnel needs, and financial needs.

SHIFTS: Full-Time Day Shifts. 40 hours/week

Responsibilities

  • Patient Care
  • Build relationships with patients and families
  • Scheduling, prepare for and attend physician renewal/new patient appointments
  • Initial Nurse Assessments and start of care visits
  • Medication Interaction checks
  • Maintain patient binder in home
  • Delegates authority appropriately and work to achieve coordination of all aspects of the company programs.
  • Ensures implementation of the company policies and procedures.
  • Ensures orientation, evaluation and continuing education for all staff.
  • Responsible for the overall supervision of all the staff in the area.

Qualifications

  • Bachelor of Science in Nursing preferred.
  • Current RN licensure in the state of NM required.
  • Qualified candidate must possess: strong sense of professionalism, terrific interpersonal skills and be a true team player.
  • Previous case management or leadership role(s) which would include; Interviewing, hiring, on-boarding procedures, mentoring and leading.
